Clinique Dentaire Girard & Simoneau is a general dental and denturology clinic located at 41, rue Gentilly Ouest, Longueuil, QC J4H 1Y9. 2. Information collected
We collect information required to deliver safe, compliant dental care: name, date of birth, contact details, RAMQ number, private insurance and/or CDCP coverage, medical and dental history, medication, allergies, X-rays, clinical photographs, treatment plans, and chart notes. On this website, we also collect what you submit through the contact and appointment forms.
3. Purposes
Information is used to: (a) deliver, plan and coordinate care; (b) bill services and submit claims to applicable plans (RAMQ, CDCP, private insurance); (c) send reminders, confirmations and follow-ups; (d) meet legal and ethical obligations applicable to dentists and denturists; (e) respond to requests submitted via this site.
4. Professional secrecy
Under section 60.4 of the Code of Professions and the Code of ethics of dentists (RLRQ c D-3, r 4), information about you is protected by professional secrecy. It cannot be disclosed without your consent, except in cases provided by law.
5. Sharing with third parties
Your information may be shared with: (a) your insurers with your consent; (b) dental laboratories to produce dentures, crowns or appliances; (c) other health professionals on referral or with your consent; (d) Health Canada / CDCP for billing and plan compliance. We do not sell your information.
5b. Technical sub-processors of this website
The website uses the following technical services, which never have access to your dental record (the dental record remains in the clinic's sole custody under the Professional Code and the ODQ's record-keeping regulation):
- Cloudflare (United States / global edge) — static hosting (Cloudflare Pages) and operational technical logging (anonymized access logs).
- Aspyn AI Inc. (Longueuil, Quebec) — secure relay of form submissions (appointment requests + contact) to the clinic's internal email recipients (the forms.aspyn.ai service). No clinical data is processed.
- Cloudflare Turnstile (Cloudflare Inc.) — privacy-respecting anti-bot verification, with no persistent cookies and no advertising-profile tracking.
- Google Tag Manager + Google Analytics (Google Ireland Ltd. / Google LLC, United States) — tag management and audience measurement. No tag fires until you give explicit consent via the cookie banner (Google Consent Mode v2 default-denied). If you decline or do not respond, no data is transmitted to Google.
6. Retention
Under the Regulation respecting the keeping of records of members of the Ordre des dentistes du Québec, dental files are retained for at least five years after the last entry. X-rays and photographs follow the same rule.
7. Your rights (Law 25)
You have the right to: access your file, request rectification, withdraw consent (subject to applicable professional duties), know which third parties received your information, and file a complaint with the Commission d'accès à l'information du Québec.
